How to submit a permit ammendment

The submission of a permit amendment is available through the secure web portal. The spokesperson or their delegate may submit permit amendments as required to identify their upcoming activity.
Experiments may be scheduled for multiple visits, therefore, prior to each visit, researchers are requested to review their permit and make the necessary amendments to identify the planned activity for the upcoming experiment.
Amendments to materials/samples/equipment or ancillary facilities requires REVIEW and APPROVAL prior to being used. Approved amendments result in an Amended Permit. Unapproved or In Progress amendments result in the last valid permit being applicable. Four weeks are requested for reviews of Permit Amendments prior to the scheduled experiment. Amendments submitted after the four weeks are not guaranteed to be processed and the last valid permit will be used for the experiment.
The permit form will be a display of the proposal form with all tabs displaying, however, only Research Team, Safety, Equipment and Ancillary Facilities tabs will allow edits. The others will be viewable but not allow edits. The safety section for materials will have a column indicating “Bringing to CLS” and the column to “add/remove” samples. This provides the applicant with the opportunity to add and/or remove a sample as well as indicate whether or not a sample will be used on a particular visit. In some cases the list of materials is very extensive; therefore, being able to identify the specific materials for each visit is beneficial so the permit report displays items accurately on site at CLS. The column “Bringing to CLS” will be defaulted to “yes” from the initial proposal submission and the spokesperson will be able to indicate “yes/no” during a permit amendment to indicate if brining on site.
- New sample/material added – reviewed and if approved added to master listing of samples/materials.
- Sample/material previously approved and “No” is selected for bringing to CLS, then permit amendment displays the change occurred and the permit report will not display the material/sample.
- In the case the sample/material was previously approved, then indicated “No” and then on a future visit indicated “yes”, then the permit amendment review screen will displays these in a different color, which indicates, previously approved, now bringing to CLS on next visit. This will allow an expedited review for these samples/materials.
When a Permit Amendment has been submitted, and the reviews for that amendment are in progress, another permit amendment for that proposal cannot be submitted. However, once the review is completed, another permit amendment can be submitted against the proposal.
If submitted amendments are approved, the changes to the proposal indicated by the data entered in the Permit Amendment and any changes to the form during the review become part of the proposal data and data tracked.
While a Permit Amendment is in progress, the submitted amendment can be viewed by selecting “Amendment in Progress” from the “View” column from “My Proposals” for the spokesperson and from “Administrator” tab. Viewing the amendment will display the requested changes highlighted in a color.
